- a V-22 Osprey crashed on 5 April 2001 after a hydraulic line ruptured. The pilot reacted properly, but a "computer software anomaly" caused "uncommanded aircraft pitch, roll and yaw motions, which eventually stalled the aircraft."
- On 26 September 1983, the Soviet Union's early warning system reported a launch of American Minuteman missiles against the USSR. Lieutenant Colonel Stanislav Petrov suspected the software was wrong, and chose not to initiate World War III.
